About the Project
Trinity is partnering with the Center 4 Health Research at the University of Illinois College of Medicine-Peoria to launch the HEALS-Diabetes Initiative, a project that aims to reduce the healthcare burden in Fulton County by promoting low-cost, sustainable diabetes prevention and management. The HEALS-Diabetes intervention is a three-year, community-driven health education program designed to reduce diabetes risks and improve disease management for residents of Fulton County using a culturally tailored, faith-based behavioral lifestyle model. The program will empower community members to take charge of their health, thereby reducing the burden of diabetes and cardiovascular disease.

The survey is open to residents of Fulton County, Illinois with some connection to type 2 diabetes. Your response will help the project team tailor their efforts to best serve us here in Fulton County!
